Insurance Extend Consolidated System Procedures

Instructions:​

Fill in all required data and submit to insurance and pension schemes in the work place. Introducing bulletin can be used for more details regarding the required documents.

First: insured person details:

  • ​Date of birth upon the legal documents of the place of birth (country).

Second: Contributions

  • The insured who meets the terms and conditions mentioned in the guide bulletin prepared for employers are subject to the system.
  • Date of service: is the actual date of joining work (start the service)
  • Date of contribution: is the date of being subject to the system (1/1/2006) for all GCC countries except UAE (1/1/2007). In the event of work after these dates, the date of service will be based on for contributions.
  • Contribution salary: is defined upon law/ civil pension/ social insurance provisions in the home country of the employee as shown in the introductory guide of the employer (download from the following websites). A detailed salary information to clarify the salary at the beginning of contributing is required.

  • Salary at the end of service is total contribution salary attached with a detailed salary component certificate.

  • Photocopy of passport, record document all stamped by employer.
  • Appointment decision or work contract in government sector include ( basic salary+ housing allowance+ children social allowance+ cost of living if any) 
  • Appointment decision or work contract in private sector include ( monthly wages plus any monthly allowance)
  • Birth or age estimation certification


  • Photocopy of the record document
  • Disability report by the medical committee
  • Death certification
  • Work injury ( police report of the accident in the work place country)
  • Pension bank account (Bank notification)

Kingdom of Bahrain (Gov.)

  • ​Population no. is employee identification
  • Passport photo copy, population card and subject to system card.
  • Basic salary+ social allowance at the date of appointing.
  • End of service document
  • Salary scale document
  • Disability or life risk certification from the medical committee
  • Discharge or disciplinary decision document.
  • Loss document upon work place country.
  • Unpaid vacation.


Kingdom of Bahrain (Private)

  • Passport photocopy.
  • Population card photocopy
  • Death or disability documents (disability report from the medical committee.

KSA (Gov.)​

  • Civil state card
  • Appointing decision certified photocopy
  • Salary scale document.
  • End of service certification photocopy.
KSA (Private)​
  • Photocopy National document or family card for women.
  • Wages amount for periods to be calculated.
  • Work contract photocopy
  • In the event of register the entity for the first time (printed of the logo and address, written statement, e-mail by the official mail)
  • All documents are certified by work place official stamp.

  • Death certification

Sultanate of Oman (Gov.)​

  • Photocopy of appointing decision or work contract.
  • ID photocopy
  • Passport photocopy
  • Approved salary scale from the date of appointing.
  • Birth or age estimation certification


 

  • End of service decision
  • Bank account card
  • Statement of absence and unpaid suspense.
  • Death certification
  • Work injury ( police report of the accident in the work place country)
  • Resignation request
  • Employer letter of death relevancy to work.
  • Approved medical report (fit or not fit)
  • Document if there is a legal case.

Sultanate of Oman (Private)​

  • Passport or ID photocopy.
  • Work contract certified by employer in the work place country.
  • End of service certificate

  • Qatari ID photocopy
